Mood and Psychotic Disorders Clinic, Attention Deficit/Hyperactivity Disorder Clinic (6-22 years)

• Psychiatric evaluation and short-term psychopharmacologic treatment for patients currently in treatment but requesting a second opinion
Fragile X Behavioral Health Clinic (all ages)

• Persons with a diagnosis of Fragile X Syndrome and behavioral challenges
• Provide diagnostic and psychopharmacologic consultations
• Provide ongoing psychopharmacologic treatment as indicated

Please Note: If there is a question of whether a child has an Autism Spectrum Disorder (ASD) but has not been diagnosed, please make referral to Developmental and Behavioral Pediatrics in Benedict, University Hospital. Telephone: 774-442-3028.
Note: All clinics partner with the youth’s Primary Care Provider to facilitate ongoing medication monitoring.

• Comprehensive Evaluation for Neurodevelopmental and Complex
Psychiatric Disorders
- Second opinion/consultation for diagnostic clarification and treatment
recommendations
- Diagnoses, including autism and Fragile X, and complex psychiatric
disorders, such as mood and psychotic disorders
• Occupational Therapy and Speech-language Therapy
Personalized therapy services include:
- Review of Individual Education Plan (IEP) and records
- Attend school/program meetings
- School and home observations
- Family and/or provider training

CANDO Request for Services Form
For your convenience, the link to the CANDO Request for Services form is provided below. The primary care physician must complete and Fax or Email the Request for Services form to CANDO. UMMHC primary care physicians should submit an ambulatory order to CANDO via EPIC.
Physicians: Please complete the Request for Services form and fax it to the CANDO office.
Parents: Once your primary care physician has sent the Request for Services form, the request will be evaluated by our clinical staff and, if appropriate, we will contact you to schedule an appointment. If not, we will follow-up with your primary care physician with recommendations for a service that may better meet your needs.
