A "slow" monomorphic (not torsades de point.) Ventricular tachycardia at around 110. There are no P waves to suggest SVT with aberrancy. Flecainide is a classic antiarrhythmic agent with an approximately 15% risk of pro-arrhythmia effect, especially in patients with structural heart disease. Hypokalemia also potentiates pro-arrhythmia, and was the suspect in this case.
