Answer for January 28, 2013

Acute antero-lateral myocardial infarction. No Qwaves have developed yet, but "tombstone" configuration is present in V2-V4. The ST depressions are likely reciprocal changes, non-ischemic, seen with acute transmural ischemia in the antero-lateral walls. Almost always, when both ST elevation and ST depression are present together, the ST elevation is where the action is!
