Leadership Application Process

Each leadership role in Vista has a separate online application.  Use the Qualtrics application links on this page to complete the corresponding application.   Each application contains the same questions which we are providing below, although your answers should be contextual to the role(s) you apply for.  For example, your qualifications for pathways program may differ from block co-director position.  We encourage you to use a Word document to prepare your responses so you can reuse content where applicable.  

Application Questions

2. Academic Rank
3. Department
4. Attestation of support from Division Chief/Department Chair (you attest for their acknowledgment)
5. A selection process to narrow down your interest (where applicable)
6. Open Text Question demonstrating your qualifications for the role (approximately 500 Words/2500 characters)
7. Demonstration on collaboration to grow the Vista program

• Core Small Group Faculty filled on a rolling basis, next application deadline is June 1
• Openings available for Longitudinal Focus Topic Director for Health Equity, Diversity, and Inclusion (HEDI), and Patient and Provider Wellness (PPW)
• Funding support begins July 1, 2022
