Adult Diabetes Care at UMass Memorial Health

A Team-Based Model of Individualized Diabetes Care

The adult care team, led by clinical chief Samir Malkani, MD, includes endocrinologists (MDs), nurse practitioners (NPs), certified diabetes care and education specialists (CDCESs), nutritionists, licensed practical nurses, and patient care assistants who collaborate to customize care plans for individuals living with Type 1 and Type 2 diabetes.

Care Coach Program

The Care Coach Program supports at-risk patients in managing dangerously high blood sugars through regular weekly check-ins.

The Diabetes in Pregnancy Program provides comprehensive blood glucose management for women before, during, and after pregnancy. We provide care for all types of pre-gestational and gestational diabetes.
