The patient has developed left axis deviation (axis now near - 60 deg), QRS widening into the IVCD range (about 0.10 sec in this case), and ST elevations in the septal leads. The EKG diagnosis is new left anterior fascicular block, and the likely underlying cause is a recent septal MI
