Core Modules

All participants are required to take all core modules to complete the course 

  1. Introduction to Integrated Care Management in the Patient-Centered Medical Home
  2. Effective Patient-Centered Care Plans
  3. Networking in the Medical Neighborhood
  4. Fostering Patient Activation Through Shared Decision Making
  5. An Introduction to Motivational Interviewing
  6. Effective Use of Registries to Improve Care for High-Risk Patients

Note: Participants can customize their experience of the course. Successful completion of a minimum of 9 self-selected elective modules and all core modules is required for course credit. No additional credit is granted if additional electives are taken.  All modules are available for participants to view.
