In order to schedule space for the academic programs of the T.H. Chan School of Medicine, Tan Chingfen Graduate School of Nursing and Morningside Graduate School of Biomedical Sciences and Grand Rounds for Academic Year 2014/15 (July 1, 2014-June 30, 2015) complete the attached e-form by February 1, 2014.

All academic requests will be scheduled before other events. E-Forms for Academic requests can accommodate from one to sixty dates on the same form.

Each event requires two contacts, a"Requested For" contact will be able to see the reservation under My Reservations on their home page and has permission to make changes. The "Requested By" contact  can see the reservation on the schedule but does not see the reservation under My Reservations and does not have permission to make changes. These should be two different people.
