Application Process

PMHNP graduates, new hires, and students who seek to gain valuable community health experience are encouraged to apply. The online application is confidential and takes only minutes to complete. In addition to the application itself, the following documents will be required for upload:

1. Copy of your CV or resume (graduates only)
2. Copy of your Nursing License (all applicants)
3. Copy of your Nurse Practitioner Certification and NPI number if available (graduates only)
4. Official Transcript (can be in process)
5. Letter of Endorsement from your clinical placement representative (PMHNP students only).
6. MA Controlled Substance License/DEA or invitation to sit for licensure (graduates only)
7. Two letters of recommendation (graduates only, professional and/or academic)
8. A brief personal statement describing your professional goals related to seeking a fellowship with a community health center (Max: 350 words). (all applicants)

All interested individuals must apply. Once your application is submitted, UMass Chan GSN will review your information, determine eligibility, and match you to a community health center (CHC) that aligns with your interests. If you are a new hire applying for a Fellowship, your CHC employer must apply and be awarded.  No Fellowship placement is guaranteed.
