PDM puts service first: we partner with UMass Memorial Health and its clinical teams to implement, support, and evaluate technology-enabled care. Each year we take on three service projects for clinical partners, subsidized through our core, in addition to our funded portfolio.

⚡ Digital Health Clinic
Remote patient monitoring for more than 4,000 patients at UMass Memorial, with PDM providing evidence generation, optimization, and analytics.

🏠 Hospital at Home
PDM leadership helped build the Hospital at Home program. PDM fellows now practice as attendings within it, while the program continues to support its ongoing evaluation and optimization.

🌬️ Tele-Pulmonary Rehabilitation
A home-based rehabilitation model that PDM helped take from proof-of-concept study to standard of care.

🚑 Mobile Integrated Health
Paramedic-delivered care in the home, integrated with remote monitoring. Early home-based models were associated with a 60 percent reduction in unplanned escalations to brick-and-mortar facilities.

🫀 Subacute Rehab at Home
Development, implementation, and evaluation of subacute rehab at home through a RCT funded by the Commonwealth of Massachusetts

📊 Clinical Operations Support
More than 20 projects evaluating and improving programs across the health system, including evaluations of specialty pharmacy, diabetes care coaching, and advanced therapeutics, and support for UMass Memorial Health submissions for Medicaid supplemental funding.
UNITED: OUR FLAGSHIP APPROACH
UNITED is PDM's organized model for home-based clinical studies and care, built from five years of practice. It packages what we have learned into repeatable components: documented best practices and protocol "recipes" for decentralized studies, patient design studios and community advisory engagement, remote recruitment and consent pipelines, device logistics, and data pipelines that connect patient-generated data to the electronic health record. UNITED links institutional capacity for home-based studies and trials directly to clinical care, and it anchors PDM's next phase of growth. UNITED is funded by NCATS CTSA award UM1-TR001453, Element-E
