About the study
TEAM-CGM is a clinical trial designed to test if team support from clinical pharmacists and community health workers alongside continuous glucose monitoring (CGM) and telehealth can improve diabetes care and blood sugar levels in people with type 2 diabetes.
Study Details
The study will last 12 months
If you choose to join this study, you will recieve help from a pharmacist and be randomly assigned (like pulling a name out of a hat) to have or not to have a CGM and a Community Health Worker (CHW) help you with your diabetes care. You will be asked to:

- Provide a finger-stick blood sample to test your A1C every 3 months
- Use a profressional CGM at the beginning of the study, at 6 months and 12 months
- Complete surveys over the phone
- Speak to a clinical pharmacist over the phone or by video (Zoom call) every month.
- If you are assigned to use a CGM, we will ask you to use a second CGM when working with your pharmacist
- If you are assigned to work with a CHW, you will be asked to complete a 30-60min home visit with the CHW once a month.
TEAM-CGM Participant Timeline
Random Assignment
After consenting to the study you will be randomly assigned to receive either:
- Pharmacist and CGM OR
- Pharmacist-only care
After 6 months, you will have an A1c (average blood sugar level) test done to see if your blood sugar levels are at goal.
If not, you will be randomly assigned a second time. If you first received Pharmacist and CGM, you will be randomly assigned the second time to:
- Pharmacist, CGM, and Community Health Worker OR
- Continue with Pharmacist and CGM
If you first received Pharmacist only care, you will be randomly assigned the second time to:
- Pharmacist, CGM, and Community Health Worker OR
- Pharmacist and CGM
If your A1c test at 6 months shows your blood sugar levels are at goal, you will continue with your routine diabetes care, and not be randomly assigned a second time. This is called the maintenance phase, see the diagram above.
Frequently Asked Questions (FAQs)
The TEAM-CGM research study is designed only for patients with Type 2 Diabetes. Unfortunately, if you have Type 1 diabetes, you cannot participate in the study.
No travel is required on your part. You can conduct all the study activities, including the pharmacists visits from home.
We will send kits to your address for you to collect a sample for your A1c, and our data collectors will call you to answer questionnaires.
If you are assigned to recieve a Community Health Worker, they will schedule a time that works best for you to do a home visit.
In this study the Community Health Worker (CHW) is responsible for helping patients and their families navigate and access health and community services whiling adopting healthy behaviors. The CHW is part of the care team and may help participants with the following tasks:
- Coordinating telehealth visits
- Assist with setting up CGM including reviewing the results
- Reinforce and clarify pharmacists recommendations and plans
- Provide health education
- Help participants set up and navigate the patient portal
- and more!
You will be paid $245 to $270.
$25 for each A1c data collection (5 time points)
$25 for an 18-month A1c( for a random selection of ~75% of study participants)
$40 for the 10-day professional CGM (3 time points)
