Longitudinal Experiences

Longitudinal Experiences Header.jpgPrimary and Consultation Geriatric Medicine Clinic

Fellows spend one half-day per week under the supervision of a UMass Chan faculty member in the geriatric medicine ambulatory clinic. Fellows participate directly in initial and follow-up evaluations of patients > 70 years of age and develop a panel of patients they follow throughout the fellowship. 

Long-Term Care

Fellows spend one half-day per week following a panel of 10-12 long term care patients with a Geriatric Medicine faculty preceptor at a local skilled nursing and rehabilitation center.

Home Care 

Fellows follow a small panel of home bound patients, making home visits with supervising attending physician, nurse practitioner and independently.

Program for All-Inclusive Care of the Elderly (PACE)

Fellows spend a full day every other week at one of two local Summit ElderCare PACE sites. They work with an interprofessional team that provides adult daycare, home care, hospital and sub-acute care of its program participants. The fellow attends the daily team meeting and sees their patients at the day program site and does home and sub-acute care visits.
