Complete Your I9

Option 1:
In order to complete your I9 Employment Authorization, please follow the steps listed below:
- Complete Section 1 of the I9 through the New Hire Portal.
- You will then need to meet with Human Resources to present original and unexpired form(s) of government-issued photo ID(s) at:
» 333 South Street, Shrewsbury, MA 01545
- Go to the Main entrance and inform Security that you are here to meet with HR in The Learning Center.
- Open Monday, Wednesday and Thursday from 9:30 AM to 11:00 AM
Bring either ONE document from List A
(ID needs to be original and unexpired) –

- U.S. Passport or U.S. Passport Card (Please send copies of the signature page, photo page, and the last page which includes the ID number with barcode at the bottom.)
- Permanent Resident Card (Please send copies of the front and back of the card.)
- Foreign Passport containing I-551 stamp
- Employment Authorization Document
- Foreign Passport with Form I-94 arrival record

- Driver’s license or identification (ID) card issued by a state or outlying possession of the United States, provided it contains a photograph or information such as name, date of birth, gender, height, eye color and address
- ID card issued by federal, state or local government agencies or entities, provided it contains a photograph or information such as name, date of birth, gender, height, eye color and address
- School ID card with a photograph
- Voter’s registration card
- U.S. Military card or draft record
- Military dependent’s ID card
- U.S. Coast Guard Merchant Mariners Document (MMD) Card
- Native American tribal document
- Driver’s license issued by a Canadian government authority

- U.S. Social Security account number card that is unrestricted
- Certification of Birth Abroad issued by the U.S. Department of State (Form FS-545)
- Certification of Report of Birth issued by the U.S. Department of State (Form DS-1350)
- Original or certified copy of a birth certificate issued by a state, county, municipal authority or outlying possession of the United States bearing an official seal
- Native American tribal document
- U.S. Citizen ID Card (Form I-197)
- Identification Card for Use of Resident Citizen in the United States (Form I-179)
- Employment authorization document issued by DHS

Option 2:
(Completing with an Authorized Representative)
In order to complete your I9 Employment Authorization, please follow the steps listed below:
- Complete Section 1 of the I9 through the Onboarding Portal.
- Provide your authorized representative’s email address, as well as their first and last name. Your authorized representative will receive an email with instructions on how to complete Section 2 of the I9.
- Meet face-to-face with your chosen authorized representative and have your authorized representative verify the ID being used for your I9. An authorized representative can be anyone other than yourself such as a household member, neighbor, family member, etc.
- UMass Chan HR will be notified once your authorized representative has completed Section 2.
