UMass Chan Employee Health Services Clearance

Based on your access and exposure for your role, you need to provide forms to the UMass Chan Employee Health Services Department, along with your immunization records. Also, you will need to meet with the UMass Chan Employee Health Services Department. Please follow instructions on the Employee Health Services message that will be emailed to you.

You need to provide the following information to your Onboarding Specialist in order for your Specialist to schedule your appointment with Employee Health Services:

1. Your date of birth
2. Last 4 digits of your Social Security number
3. The best phone number Employee Health Services can reach you at, preferably your cell phone number
