COVID-19 Vaccination Verification

As a condition of your employment at UMass Chan, any compliance requirements listed below are mandatory. Please review and take the next steps.

To be completed prior to your start date:

• Proof of vaccination is required prior to your start date. 
• Please submit proof of vaccination to Theresa.Kane@umassmed.edu. (Please indicate that you are a NEW HIRE when submitting proof of vaccination.)
• To request a medical exemption, faculty and staff may send a request with a letter from their healthcare provider to Employee Health Services at employee_health_covid-19_mailbox@umassmemorial.org.
• To request a religious exemption, faculty and staff should email Matthew Lyford in UMass Chan Human Resources at Matthew.Lyford@umassmed.edu.
