Inpatient Diabetes Blood Glucose Management

One third of hospitalized patients have diabetes.  The UMass Memorial Diabetes Center of Excellence has revolutionized Inpatient Diabetes Blood Glucose Management and decreased length of hospital stays by 25%. 

This was accomplished by expanding care for all hospitalized individuals admitted with diabetes, as well as people who get diagnosed while in our care.  Training has increased staff awareness about diabetes related issues. 

Our Goal = Improved Blood Glucose Control

No matter why you were admitted to the hospital or emergency room, the UMass Memorial Diabetes Center of Excellence inpatient Blood Glucose Management Service team can be consulted by your care team to assist with diabetes issues during your hospital stay.  Patients using insulin pumps or concentrated insulins will be consulted to determine appropriate insulin regimens while in the hospital. 

Our focus is to improve glucose control for anyone living with diabetes who receives medical care at a UMass Memorial hospital.  Those getting diagnosed with diabetes for the first time will learn diabetes survival skills.  Prior to sending people home, medication prescriptions are coordinated and follow-up appointments are scheduled, including outpatient diabetes education.

Inpatient Diabetes Education for UMass Memorial Patients

The UMass Memorial Department of Pharmacy Services assists the inpatient Blood Glucose Management Service to determine insurance coverage and need for prior authorization for diabetes medications for discharge.  The Blood Glucose Management Service will provide or oversee basic diabetes survival skills to allow for safe discharge.  Formal diabetes education will then be scheduled as an outpatient to enhance these skills and empower patients to fully participate in their diabetes care.

Blood Glucose Management Program 

Whether newly diagnosed or currently living with diabetes, it's essential to establish a clear understanding of the disease and how to best manage it with insulin and/or oral medication.  Regardless of the reason for hospitalization, anyone living with diabetes receives the most up-to-date treatment options and diabetes education survival skills, including insulin administration and blood glucose monitoring training.  Patients are taught using their own blood glucose meters, when possible, and insulin delivery methods to be used when they return home.

The UMass Memorial Diabetes Center of Excellence offers all diabetes patients the opportunity to participate in clinical studies to help reach desired goals and outcomes.

Follow-up Care

Our inpatient diabetes care team works with your primary care physician, or if necessary, helps to identify a health care provider if you don't already have one.  Prior to transitioning home, we schedule all follow-up outpatient appointments to ensure the necessary care after your hospital stay.
