Intravenous (IV) contrast

Why do we need IV contrast?

  • Lesion detection
  • Lesion characterization
  • Vascular anatomy and patency (CT scan)

General Principles

  • All MRI scans include non-contrast sequences
  • We cannot administer IV contrast when GFR is below 30
  • Prefer not to administer contrast if there is history of anaphylactic reaction related to IV contrast
  • All CT Angiography and Venography studies (not MRA and MRV) are performed “with IV contrast”

Body Imaging

  • General survey exams for pain or other not specific symptoms including malignancy (known or suspected) or fever (infection) should be ordered “with IV contrast” and not without and with IV contrast
  • CT chest for follow up on lung nodules should be ordered “without IV contrast”
  • Common indications that do not require the use of IV contrast and can be ordered “without IV contrast”: renal colic, diverticulitis, hernias, acute appendicitis (best seen with IV contrast), Abdominal aortic aneurysm (AAA) screening
  • A “without and with IV contrast” examination is generally needed for the work-up of hematuria (CT Urogram), known Liver Pancreas and Kidney masses, aortic dissection and endovascular repair or aortic aneurysm
  • Abdominal and pelvis MRI should be ordered “without and with IV contrast”
  • MRCP examination is ordered “without IV contrast”

Musculoskeletal Imaging

  • ANY Orthopedic/Musculoskeletal examinations in the absence of a known or palpable mass, or possible infection should be ordered “without IV contrast”
  • ANY Orthopedic/Musculoskeletal examinations to assess a known or palpable mass, or possible infection should be ordered “without and with IV contrast”

Neuroradiology

  • CT Brain: order “without and with IV contrast” for assessing for metastatic disease only or if MRI is contraindicated. Otherwise, almost always order “without IV contrast”
  • CT Neck: order “without and with IV contrast” for assessing for salivary glands stones. Otherwise, almost always order “with IV contrast” only
  • CTA Head and Neck: “with IV contrast” only
  • MRI Brain: order “without and with IV contrast” for the following indications (otherwise, order “without IV contrast”)
    • Known history of cancer, metastatic disease
    • Known history of any brain mass
    • Vertigo/tinnitus (MRI IACs)
    • Known multiple sclerosis
    • Specific cranial neuropathy
    • Pituitary or hormonal imbalance (MRI Pituitary gland)
  • MRA Brain: order “without and with IV contrast” for the following indications (otherwise, order “without IV contrast”)
    • Known aneurysm
    • Known AVM
  • MRI Spine: order “without and with IV contrast” for the following indications (otherwise, order “without IV contrast”)
    • Known history of cancer, metastatic disease
    • Known history of any spinal cord mass
    • Known multiple sclerosis
    • Post operative spine
    • Suspicion of infection (Epidural abscess, discitis, ect.)
  • Always order “without and with IV contrast” for the following:
    • MRV Brain
    • MRA Neck
    • MRI Neck
    • MRI Orbits
    • MRI Brachial Plexus
