Minnesota Health Partnership- Health Care Quality Information Survey
The
Health Care Quality Information Survey
was designed as a part of the
Minnesota Health Partnership's Coordinated Health Care and Disability Prevention Project.
It was used as a data collection tool to help evaluate the project's outcomes. This survey was used to establish a baseline level of health and functioning from which follow-up surveys could be used to assess changes resulting from the health care and disability management interventions used.
