1:00 The Development of the Coordinated Health Care and Disability Prevention (CHCDP) Model-  Marilou Calasanz

The development, market demand and core tenants of the model including employer-work site changes, provider practices and employee/patient education will be discussed.

1:15 Evaluation of the CHCDP Model...Presentation of Results-
Bryan Dowd, William Lohman, Michael McGrail, David Radosevich

Researchers will present findings related to the impact of CHCDP on clinical outcomes and patient satisfaction, impact on providers and employers, and the economic influence of this model on health care and disability costs.

3:15 Panel Discussion- Jon Christianson, Facilitator

The panel will feature clinic and employer representatives who were instrumental in the implementation of the CHCDP model. Different perspectives will be provided on key issues related to the experience of MHP and what can be learned for the future of disability prevention practices and the delivery of health care.
