FaCES Program Design
Design Focus
The FaCES program was designed to address three problems:
- Non-compliance with state and national recommendations for health screenings and exams for children in foster care
- Absence of integrated case management by providers to ensure prompt evaluation and referral follow-up; coordination and information exchange with previous providers; and ongoing communication with DSS and foster families
- Lack of uniformity in both care delivery and provider understanding of complexity of the medical and developmental issues among these children
Program Components
A treatment team of individuals experienced in dealing with the needs of the foster care population was assembled to provide the following:
- Coordinated intake process
- Communication with existing or past medical providers
- Facilitated Access
- Timely completion of screening evaluation and comprehensive medical exam. (View Visit Components)
