FaCES Clinical Database
Development
The FaCES database was developed over a two year period of time to accomplish the following objectives:
- Provide an electronic record of each child’s clinical history and FaCES visits
- Create a medical passport to transfer relevant information to each child’s ongoing medical home
- Provide a mechanism for monitoring compliance with program policies and procedures
- Serve as a primary source of data retrieval for program evaluation purposes
Components
The database comprises the following key elements:
- An input screen for recording general placement info and FaCES visits data
- An input screen for recording the birth family and child medical history
- An auto-generated Medical Evaluation Summary (MES)
- Administrative reminders of pending items such as medical records and, referral reports, visits needing to be scheduled, MES reports needing completion, reminder calls and cases ready to close
- A record of all medical records requested and received, MES reports sent, visit reminder calls made and HIV screening requests
- A record of all referrals made and all follow up actions taken
- A wide range of statistical reports on items such as new diagnoses, medication disruptions, referral types, placements and visits by age
