Healthy Community Access Project
In FY07, Office of Community Programs (OCP) awarded $50,000 through an RFP process to each of the following networks to support the continuation and/or expansion of local collaborative associations and networks that were originally established as part of federal Healthy Community Access Projects grants.
- The Northern Tier Healthy Communities Access Project Coalition, which consists of four partners from health care and service agencies within Franklin County and the North Quabbin area. Funding supports the creation of an extended system of enrollment/eligibility assistance sites and an assessment of the impact of the new Chapter 58 “Connector” insurance programs on the low-income population within this rural area.
- The Community Health Connections Family Health Center in Fitchburg, in partnership with nine social service agencies located in Leominster, Fitchburg and Gardner. Funding supports network expansion to include schools and other community youth programs, continued outreach initiatives with Heywood Hospital and Health Alliance Hospital to uninsured and underserved residents with a specific focus on the Spanish and Portuguese, and the production and distribution of multicultural outreach and health education materials.
- A Berkshire County consortium, led by Community Health Programs (CHP) in Great Barrington and including Berkshire Health Systems (BHS), and the Brien Center for Mental Health and Substance Abuse Services (MH/SA). Funding supports an enhanced role for the Brien Center and improved access through the continued use of Mobile Outreach Treatment Teams with emphasis on chronic disease prevention and treatment, MH/SA counseling and treatment, and oral health services.
