The following are a series of evaluation forms that can be utilized in the clinical performance of the student. The weekly evaluation form may be completed biweekly, and it may be used as a mid-rotation evaluation.

Weekly Evaluation Form

Clinical Imaging Exam
This evaluation may be used to pass competency in a given nuclear medicine procedure.

I 131 Competency Evaluation

The student will observe and answer specific questions that are contained in this form.

End of Rotation Evaluation
This form must be completed by the coordinator to establish a grade for the student's clinical performance.

Immediate Student Reporting Form
If there is a problem with the student's performance, this form is to be submitted immediately to remedy any problem.
