Complex Case Management


Parents of children with serious medical conditions face countless challenges in coordinating medical services for their child. States face an equal challenge in providing them with appropriate, high quality and cost effective long-term care services. The Complex Case Management program, operated by the Commonwealth Medicine division of the University of Massachusetts Medical School, has developed solutions and Best Practice models that meet these challenges.

The Complex Case Management (CCM) program, known in Massachusetts as Community Case Management, was developed in conjunction with Massachusetts Medicaid to serve children with complex medical needs and their families. It provides states with Best Practice models to help parents care for their children in the comfort and security of their own home whenever possible. CCM program consultants help state agencies develop vital partnerships with all relevant agencies, and create comprehensive service delivery plans to best serve the needs of the child. This model helps parents access a range of long-term care services to support the child's long-term care needs. 

Our program includes models that arrange clinical and other resourcesto alleviate parents of administrative burdens, while ensuring the highest quality, most cost-effective and comprehensive care for the child. With a focus on quality, compassion, efficiency and cost-effectiveness, we assist states and medically complex children and their families in realizing the potential of comprehensive care.
