Person-Centered Services


In public health care, one size often does not fit all. Providing quality community-based services to vulnerable populations requires a person-centered approach that adapts to meet the need of each individual. With this goal in mind, The University of Massachusetts Medical SchoolÕs Commonwealth Medicine division offers a number of original programs to public sector agencies seeking to improve community-based services.

Our Community Case Management program provides public agencies and families of medically complex children with a single entry point to coordinate medically necessary, community-based, long-term care services. Nurse case managers from the program work directly with families to customize services and ensure the highest quality, most comprehensive care for their children at home.

Our Nursing Home Initiative program helps public agencies provide coordinated supports to individuals residing in nursing facilities, to maintain their functioning abilities, increase their independence and improve their quality of life. Using a person-centered approach, Nursing Home Initiative case managers determine the specialized services from which each individual will benefit most, then arrange the supports needed, provide ongoing oversight and continue to advocate on the individualÕs behalf.

Our Transitional Case Management (Trans CM) program works in partnership with state agencies to transition certain individuals with developmental disabilities from nursing facilities into community living settings. Through coordination of services to remove barriers to community living for nursing home residents and using a person-centered approach, Trans CM staff work with these individuals to access appropriate housing, financial support, medical services, equipment, home modifications, and personal care workers.

Our community-based services are complemented by the clinical and academic resources of the University of Massachusetts Medical School, as well as pioneering research in the field of long-term care and community-based services from our Center for Health Policy and Research.
