How Do I Register

You may request the Anatomical Gift Program registration packet as follows:

1. Dial 508-856-2460 if we are unavailable press 2 on your keypad and leave your name, mailing address and the number of packets you wish to receive.

2. Email ummsagp@umassmed.edu with your name, mailing address and the number of packets you wish to receive.

3. Download and print the donor registration packet.
