Health Clearance Form - The health clearance form is required for all incoming students. Please download and follow the instructions outlined on the form. This form must be completed prior to the start of class and returned to SHS to be medically cleared to start class/clinical. An e-mail will be sent by Student Health to direct you when the form is to be completed. The mask fitting process fits students with special masks to protect them from patients with droplet spread infections including Tuberculosis. Please download this form and return to SHS. Download form.Blood Borne Pathogen Policy Form (BBP) - This form explains the BBP policy developed for both UMASS Medical School and UMASS/Memorial Health Care. This policy has been developed as a means to protect both students and patients from acquiring a blood borne pathogen. Signature of this form by the student means they have read and understand this policy and their responsibilities. Please download, read it in its entirety, sign, and return to SHS for your file. Download form.TST Form - This form must be completed for annual TST testing for ALL GSN and School of Medicine students. An e-mail will be sent by Student Health Services directing you to fill out this form during the annual TST clinics (held each spring). If you have had a positive TST in the past, you do not need to attend the TST clinics. However, you must answer the first 2 questions and symptom review section and return to Student Health each year. **NOTE: GSBS students are not required to have annual TST testing.
