LOGO CONTEST
FOR THE UMass Medical School Wellness Initiative

 Create the winning logo that encourages wellness in our UMMS community and you will win a gift certificate to Trader Joe's

The Wellness Initiative:

• Encourages employees and students to increase physical activity most days of the week
• Promotes an environment that improves access to and provides options for healthy food choices in the workplace

Contestants: to enter the contest, you must be:

• 18 years of age or older
• An employee or student of UMass Medical School

How to Enter

• E-mail your logo design as a jpeg to: work-life@umassmed.edu no later than Monday, May 17, 2010 at 5 p.m.
• Please provide your UMass Medical School e-mail address, department and daytime phone number.

Content

• The Wellness logo must include the words UMMS Wellness.
• The UMass Medical School logo must remain intact.
• The Wellness logo should compliment the UMMS logo.
• The Wellness logo should be suitable for many formats including letterhead, buttons, website, fabric, posters etc.
• All submissions must be in jpeg file format.

Selection

• The Wellness Task Force will review all submissions and select the logo of choice by Tuesday, June 8, 2010
• All individuals who submitted logos will be notified of decision by e-mail.
