Requisitioning Stem Cell Lines from the Massachusetts Stem Cell Bank
In order to obtain a cell line from the Bank, the following documents are required:
1. A completed Requisition for Stem Cell Lines form from the Massachusetts Stem Cell Bank.
2. An approved UMASS Materials Transfer Agreement (MTA) and intellectual property documents for the requested cell lines.
3. A statement of expertise in handling human embryonic stem cells and/or reprogrammed stem cell lines.

Submitting a Cell Line to the Massachusetts Stem Cell Bank
To submit a pluripotent stem cell line for banking, the following information is requested:
1. Completed Application for Submission of a Cell Line
2. Detailed cell line derivation protocol
3. Culture and freeze-thaw procedures
4. Sterility and pathogen testing methods (and results)
5. Cell line characterization results, such as:
• Marker expression
• Karyotype
• Fingerprint (Genotype)
• Extended culture testing
• HLA Typing
6. A list of publications and technical reports for the cell line.
7. Relevant provenance documentation, including:
• Organ/tissue donor’s blank consent form
• Donor’s infectious disease screening results, if available
• Proof of IRB or equivalent approval
• Statement of approval from ESCRO Committee
