Intake, Assessment and Treatment Planning for Tobacco Cessation
The Center has developed a guide and training session to assist clinicians in conducting a thorough intake and assessment interview with a patient who smokes. The information can be accessed in three ways:
- The Clinician’s Guide and sample assessment form can be viewed here
- In-person Training: This training can be conducted in an interactive format for groups of providers. Call for cost and scheduling information.
- Online Training: This free online course covers the Clinician’s Guide including tips on how to conduct an assessment using a case study format
Course Objectives: The course is designed for primary care physicians, nurse practitioners, physician’s assistants and RNs. After completion of this course, participants will be able to:
- Describe how to conduct an intake interview to assess a person’s smoking history in order to determine the level of nicotine dependence and appropriate recommendations for pharmacotherapy.
- Identify the psychological, environmental and social factors that have maintained a person’s smoking or influenced their desire to quit; determine readiness to quit and assess strengths and barriers that will target quitting strategies.
- Develop a treatment plan and/or referral based on the information gathered during an initial assessment identifying the client’s level of nicotine dependence, readiness to quit, strengths and barriers to quitting, and any relevant medical or psychiatric history.
